Vet Consent & Appointment Request

Before any treatment can start, each patient needs consent from their vet, as it is legally required by the Veterinary Surgeons (Exemptions) Order 2015. We also require consent for any maintenance sessions as well. In order to obtain vet consent, please fill out the online form below, or you can drop us a message/email/phone with the details mentioned below. We will then contact your vet directly to request consent. Once granted we can organise appointments. Please be aware of our Terms & Conditions linked here and are only associated with using ER Vet Physio. These are different to Bluewaters Canine Centre's, who have separate terms and conditions for those also wanting canine hydrotherapy.
